At Ellenville Regional Hospital, we believe that every patient matters, and your well-being is our top priority. Our Patient Care Planning Program—also known as Case Management—ensures that your care is personalized, well-coordinated, and centered around your individual needs and preferences.

Whether you are receiving care as an inpatient, outpatient, or through our Emergency Department, our Case Management Team is here to guide and support you every step of the way.

What We Do

Our experienced Case Managers serve as your care advocates, working to streamline communication, remove barriers, and ensure you have everything you need for a successful recovery.

We will:

• Meet with you upon arrival to assess your needs and introduce you to the care planning process

• Stay connected throughout your treatment to ensure you and your family are informed, supported, and empowered to make confident healthcare decisions

• Coordinate your discharge plan, including follow-up appointments, home care, and therapy services

• Ensure seamless transitions for patients moving into our Sub-Acute Rehabilitation Program

• Arrange essential post-discharge services, including:

  • Home care nursing

  • Medical equipment and supplies

  • Transportation

• Collaborate with your insurance provider to help you understand your coverage and minimize delays in your care

• Facilitate communication through Language Line interpretation services, and help relay important medical information to your family and support network
